VET RECORDS

To fast track booking our services please upload your pet's vaccination records including Rabies, DA2PP, and Bordetella. According to the Department of Health, a valid certificate of vaccination must include lot #, manufacturer, expiration date and vaccination date for Rabies and the given/expiration date for DA2PP and Bordetella.

This certificate must also be signed by your pet’s Veterinarian. If you’d like your vet to fax over your pet’s shot records,

Our system will send you an email or a text message reminder 2 weeks prior to the expiration of any of your pet’s vaccinations. We cannot contact your vet on your behalf to obtain these records as each patient’s medical files are confidential.
